Communications by email, phone and text will be billed to your insurance. You maybe responsible for all or part depending on your coverage plan.             

Services may be covered in full or in part by your health insurance or employee benefit plan. Please check your coverage carefully by asking the following questions:

What is my deductible and has it been met?
Do I have a co-pay? If so, how much is it?

Cancellation Policy
If you do not show up for your scheduled appointment and you have not notified us at least 24 hours in advance or for Monday appointments by 5 pm on Friday, you will be charged your appointment fee  ( your insurance will not cover this ).
